Your initial visit will often consist of a comprehensive examination with Dr. Gordon that allows you the opportunity to discuss your dental concerns and oral health goals. Or your first visit may be a scheduled dental hygiene visit with one of our registered dental hygienists. We will review your medical and dental histories and gather the necessary diagnostic information to help formulate a treatment plan. Dr. Gordon will then spend time with you, one on one, explaining his diagnosis and treatment options. Occasionally, treatment can be done the same day as the consultation. However, a complex medical history or treatment plan will require further evaluation and a second appointment to provide treatment on another day.
Please assist us by providing the following information at the time of your consultation:
- A completed Health History/ Patient Information Form
Any past dental X-rays
A list of your current medications including any vitamins and supplements.
- If you have dental benefits, please bring your card and any associated documents
IMPORTANT: A parent or guardian must accompany all patients under 18 at the time of first visit.
Please alert the office if you have a known medical condition that requires pre-medication with antibiotics before dental treatment.
If you have had previous dental X-rays taken, you may request that they be forwarded to our office. We utilize cutting edge Digital Radiography, so the X-rays can be emailed directly to our office and stored in your chart. If additional X-rays are necessary, they can be taken at our facility. Our digital X-rays reduce exposure by 60-80% compared to traditional film X-rays.
